Carpal tunnel release is a surgical procedure used to treat carpal tunnel syndrome when symptoms such as numbness, tingling, pain, or weakness in the hand persist despite non-surgical treatments. Before surgery, patients are typically evaluated with a physical exam and, in many cases, nerve conduction studies to confirm compression of the median nerve at the wrist. Surgery is usually recommended when symptoms are severe, long-standing, or associated with muscle weakness or nerve damage, as continued compression can lead to permanent loss of function. The procedure works by cutting the transverse carpal ligament, which forms the roof of the carpal tunnel, to relieve pressure on the median nerve. It is most often performed as an outpatient procedure using local anesthesia, sometimes with sedation. In an open carpal tunnel release, the surgeon makes a small incision in the palm to directly access and divide the ligament. In an endoscopic or ultrasound-guided release, one or two very small incisions are used, and a camera or imaging guidance helps the surgeon cut the ligament from underneath the skin. In all techniques, the goal is to increase space within the tunnel so the nerve can recover. After surgery, the hand and wrist are usually placed in a bandage or splint for one to two weeks while the incision heals. Patients are encouraged to gently move their fingers early to prevent stiffness, though gripping and forceful use of the hand are limited initially. Pain and swelling are common at first and typically managed with elevation and oral pain medication. Stitches are removed at a follow-up visit, and hand therapy may be started to restore motion, strength, and coordination. Sensation often improves gradually over weeks to months, while grip strength can take several months to fully return. Recovery time varies, especially if the nerve was compressed for a long time before surgery, but most patients experience meaningful symptom relief and improved hand function.
