Egg freezing, also known as oocyte vitrification, is a fertility preservation procedure in which a woman’s eggs are retrieved, rapidly frozen, and stored for future use. The process involves controlled ovarian stimulation, egg retrieval through follicular puncture, and vitrification in a specialized laboratory. Eggs are stored in liquid nitrogen tanks at –196°C, where cellular metabolism stops completely, allowing eggs to be preserved indefinitely without loss of quality. The procedure begins with a specialized evaluation of ovarian reserve, including anti-Müllerian hormone (AMH) testing, antral follicle count, and baseline hormonal analysis. Egg freezing is ideally recommended before the age of 35 to optimize both egg quality and quantity, with the highest potential typically between ages 20 and 30. However, women up to 38–40 years old may still benefit, recognizing that egg quality gradually declines after age 35. Once a patient is deemed a good candidate, a personalized ovarian stimulation protocol is initiated. Over approximately 10–15 days, hormonal medications are used to promote the development of multiple follicles. Response is closely monitored through transvaginal ultrasound and hormonal blood tests, and medication doses are adjusted to optimize egg yield while maintaining safety. Egg retrieval is performed as an outpatient procedure under conscious sedation, so patients do not feel pain during the procedure. Using ultrasound guidance, the ovaries are accessed vaginally and follicles are aspirated to collect mature eggs. Mild post-procedure discomfort similar to menstrual cramps may occur and typically resolves within 24–48 hours with standard pain relief. Following retrieval, eggs are vitrified using advanced freezing techniques that prevent the formation of ice crystals, which is critical for maintaining egg integrity. This method results in high survival rates after thawing (devitrification). The vitrified eggs are then stored securely in liquid nitrogen tanks under continuous monitoring. The number of eggs recommended for freezing depends largely on age. For example, around age 30, approximately six eggs may be sufficient to generate a viable embryo, whereas at age 40, 15–20 eggs may be needed to achieve similar chances of success. Importantly, ovarian stimulation and egg retrieval do not negatively affect natural fertility or accelerate menopause, as the eggs retrieved are those that would normally be lost during that menstrual cycle. Egg freezing offers flexibility and future planning, allowing patients to decide when to use their eggs for pregnancy. Storage can be maintained long-term with annual plans, providing reassurance that eggs are preserved at their optimal quality until the patient is ready.
