Inlays and onlays are custom-made dental restorations used to repair teeth that have moderate decay or damage that is too extensive for a standard filling but not severe enough to require a full dental crown. They are often referred to as “indirect fillings” because they are fabricated outside the mouth in a dental laboratory and then permanently bonded to the tooth. Their purpose is to restore strength, function, and natural tooth shape while preserving as much healthy tooth structure as possible. The main difference between an inlay and an onlay lies in how much of the tooth they cover. An inlay fits neatly within the grooves of the tooth’s chewing surface and sits between the cusps (the raised bumps on the tooth). It is typically used when decay or damage is confined to the center or sides of the tooth. An onlay, sometimes called a partial crown, extends over the chewing surface and covers one or more cusps. Onlays are chosen when the damage is more extensive or when the cusps need reinforcement to prevent future fractures. The process begins with the dentist removing decay and shaping the tooth to receive the restoration. An impression or digital scan is then taken and sent to a dental lab, where the inlay or onlay is crafted from durable materials such as porcelain, composite resin, or gold. While the restoration is being made, a temporary filling may be placed. At a second visit, the custom inlay or onlay is carefully fitted, bonded to the tooth with strong dental cement, and adjusted to ensure a comfortable bite. Inlays and onlays offer several advantages over traditional fillings. Because they are custom-made, they provide a precise fit, excellent durability, and a strong seal that reduces the risk of future decay. Porcelain and composite options blend naturally with surrounding teeth, while gold offers exceptional longevity. Overall, inlays and onlays are a conservative, long-lasting solution that strengthens the tooth and restores normal chewing function without the need for a full crown.
