Morton’s neuroma excision is a surgical procedure used to treat persistent nerve pain in the forefoot when conservative treatments such as shoe changes, orthotics, or injections have failed. Before surgery, patients are evaluated to confirm the location of the affected nerve—most commonly between the third and fourth toes—and to rule out other causes of forefoot pain. The procedure is typically performed under general anesthesia, often with a local nerve block to reduce postoperative discomfort. During surgery, the surgeon makes a small incision, most often on the top of the foot, although in some cases the incision may be placed on the bottom of the foot depending on anatomy and surgeon preference. The thickened, irritated portion of the nerve is carefully identified and removed. The incision is then closed, and the foot is wrapped in dressings designed to protect the area and minimize swelling. Removing the nerve permanently eliminates the source of pain but results in a small area of numbness between the affected toes, which most patients find far less bothersome than the original symptoms. After surgery, patients can expect swelling, bruising, and some pain for the first few days. Elevating the foot as much as possible during the first one to two weeks is essential for comfort and healing. Dressings must be kept clean and dry, and walking should be limited to necessary movement only, often with the help of crutches or a walking aid. Follow-up typically occurs about 10 to 14 days after surgery, when dressings and stitches are removed and healing is assessed. Most patients return to regular footwear gradually, resume work once the wound has healed, and avoid strenuous activity or sports for several weeks. Long-term outcomes are generally very good, with lasting pain relief and minimal impact from the expected numbness.
